Actualités en infectiologie dans la mucoviscidose

Résumé

This review focused on the news in CF airways infection. International guidelines were provided for the care of non tuberculous mycobacteria, and recent studies stressed on the benefit effect of azithromycin or combined antibiotics. The identification of multiresistant environmental bacteria in airways made to account for little-known consequences. Early diagnosis and eradication of Pseudomonas aeruginosa and Staphylococcus aureus methi-R were still a concern, and reports were proposed. However, the studies on staphylococcus methi-R should be interpreted as regards the European or American continent. Thus, levofloxacine has demonstrated its efficacy without enhancing the efficiency. This drug will increase the choice for treating the patient, but no study were provided on the expected modification of the patient microbiota and the known risk of emergent resistance to antibiotics. Lastly, this review underlined that the CF practitioner was encouraged to search and not underestimate the presence of fungus, of which the not so well studied Aspergillus fumigatus.
Cette revue fait le point sur les actualités d'infectiologie dans la mucoviscidose. Pour les mycobactéries non tuberculeuses, des recommandations internationales ont été publiées, l'azithromycine est protecteur et constitue un des traitements de choix, la recherche s'intéressant à de nouvelles associations. L'identification de nouveaux germes d'environnement multi-résistants colonisant les bronches des patients alerte sur les mesures à prendre. Le diagnostic précoce de la colonisation à Pseudomonas aeruginosa reste une préoccupation de même que son éradication. La lévofloxacine inhalée peut enrichir le groupe des antibiotiques utilisés dans les infections chroniques sans être révolutionnaire et sans étude pour l'instant des conséquences attendues et décrites dans les années1990 sur l'écosystème des patients. Les études sur Staphylococcus aureus résistant à la méthicilline doivent être prudemment interprétées selon le continent où elles ont été réalisées, les souches étant différentes en Europe et aux États-Unis. Des retours d'expérience de schémas thérapeutiques pour obtenir l'éradication sont proposés. Enfin, les champignons apparaissent comme une préoccupation grandissante et leur pathogénie serait sous-estimée.
