Processus d’aide professionnelle ergonomique et cognitive avec un accompagnateur du médecin généraliste : innovation en France en matière d’aide médicale à des populations vulnérables dans le cadre du dépistage systématique du cancer

Abstract : This contribution aims to show the problems in communication and understanding during professional interviews of general practitioners who have to practice a proposal for routine screening for colon cancer. In terms of cognitive ergonomics, the doctor has to deal with cognitive overload in many ways when confronted with the obligation to give instructions regarding routine screening for colon cancer. We will discuss this cognitive overload and show the establishment of an innovative multidisciplinary project aimed at countering this obstacle by introducing a screening guide : a patient navigator, which is supposed to remedy the cognitive overload of the doctor and propose an using cognitive understanding for socially vulnerable populations (LLN, migration, unemployment ...).In situations of optimal functioning with a time taken for this specific dialogue, everything could be alright, but we have observed in a preliminary feasibility study that in this specific time there did not exist in objective reality of the physician's practice. Hence the need for care performed by a third person, this guide who is an additional screening and paves the way for a communication and a specific management of these populations. Our multidisciplinary project (epidemiologists, economists, psychologists) proposes to consider psychosocial and psycho-cognitive factors that slow or inhibit the approval of entry into the screening process. 2000 questionnaires were sent and built by this team of multidisciplinary researchers, who helped set up the same questionnaire. We constructed a grid quantitative and qualitative analysis in psychology and cognitive ergonomics : the grid has help clarify the effectiveness of this innovative approach.
